With a significant taper and several ectatic segments this case would present a challenge for traditional balloon expandable stents to obtain good apposition, the unique Self-Apposing®
nature of the Xposition S stent adapts to different vessel diameters. The recent availability of a new 37mm length allows for the simple treatment of this complex lesion with just one stent.
Severely Tapered LAD after CTO recanalisation
Dr Weilenmann, St Gallen, Switzerland, shares a case that demonstrates the need to have a Self-Apposing stent on the shelf. In this case, a straight forward revascularisation of a CTO revealed a severely tapered LAD with a large mismatch in calibre between the distal and proximal vessels.

Dr Amoroso from OLVG, Amsterdam, shares a case of STENTYS Xposition S stents being used to treat ostial and distal lesions in a Saphenous Vein Graft. The small cell area and gentle outward force of a self-apposing stent are suited to heavily thrombosed vessels and may reduce material prolapse and distal embolisation. The active expansion of the stent allows for apposition to be maintained as debris are squeezed or as the lumen increases even in vessels upto 6mm in diameter
